You would have to jailbreak the iPhone to use custom themes. Even though it is very easy to JB the iPhone, think long and hard before doing it. It does make the iPhone less stable and can cause it to crash. If you do decide to JB your iPhone, read the procedures more than once including the warnings. Also, do it yourself and don't let someone do it for you because your friend may not be around if things go wrong.
That being said, I am enjoying my JB'ed iPhone. I've got it set up so that I can have 25 icons instead of 16 on each home screen plus 5 or 6 icons in the dock. Also, I can run Internet radio and ambient noise apps in the background while I run other apps. Also, the Cydia app called SBSettings allows you quick access to 3G, Bluetooth, WiFi, and Location toggles just by swiping the top status bar. SBSettings also allows you to add apps, a second mini dock and other things. Personally, I don't care about themes in general, but I do like to hide the icon labels, change the lockscreen lock, and do other minor stuff.

I'm new to the iPhone and heard that you can install themes to change the look. I figured before I started messing up the iPhone, I should check with you guys.
Any recommendations on how I can change the themes? I don't want to just start downloading stuff blindly.
