What command line does everyone use for Lame? I use --alt-preset cbr 192, seems to produce superb results from WAVE source.

forgot to mention, I use HydrogenAudio's modified Lame 3.90.3

"alt preset extreme" here. I'm happy with the results. I haven't decided if any AAC settings can top the Lame encodes at comparable file sizes. The jury is still out...

You use HydrogenAudio modified codec, yet you dont take their advice. CBR is lower quality than a VBR mp3 :P
Im Into OGG now, i'll rip mp3 for iPod @ abr of 256.
