Transmitter. Just find a frequency that is free on both ends of it.

I used both. I finally ended up with the cassette adapter for the car and the FM transmitter (iTrip) for using my iPod with my home receiver.
With the iTrip in the car, I had problems finding a freq that was clear in all parts of town. If I drove across town, I'd get station interference in different areas on different freqs. Also, for some reason, the iTrip frequently lost its channel programming and I would have to reset it.
The default freq for the iTrip is clear at my house, so it works well there.
I also get, IMHO, a cleaner sound using the cassette adapter in the car. Now, I happened to have a few of those cassette adapters on hand and some of them made quite a bit of mechanical noise while in the tape player in the car. One was completely quiet so, obviously, that's what I'm using.

I used a cassette adapter until the player in my car broke. Then I bought the iCarplay by Monster cable, and the sound quality was horrible in comparison. And I also find it hard to find a clear station while driving to another town.

blink4life90,
There's pros and cons to both methods.
Cassette adapter:
Pros: Great sound. Not having to deal with finding radio stations. No static noise.
Cons: Wires. You might also experience a hum through your speaks when plug into a cig adapter.
FM transmitter:
Pros: One wire. Works on almost all cars ( no need for tape deck).
Cons: Ok sound. Good enough for most folks, but it wasn't good enough for me.
It all about what's important to you. Sound or convenience.
