Does it scratch easily?

I'm getting a touch (16GB 2nd gen refurb) in a few days, and I was wondering if it was safe to have it in my pocket with, for example, keys or a cell phone; also, do you think I should get a screen protector or case or should I not bother?

If it's going in your pocket with keys and other stuff, I'd say get a case or at the very least a stick on film
protector.

I carry mine in a shirt pocket with nothing else, and my (unprotected) screen is still OK after about 7 months.

I have a screen protector on mine as well as a film on the back. I used to keep it in a case as well, but I don't bother anymore. The edges that are now covered by the film on the back are scratched up a bit just from everyday use. I would at least get a film for the screen. I have the power support anti glare set and am pretty happy with them.

I'm taking good care of my gadgets. The back might scratch if you put it in with keys. I have that Case-Mate Vroom on mine to prevent from scratches.
