I have $40 Sony buds that are OK, but then got Panasonic RP-HJE300s for much less a couple of weeks ago - I like the sound very much and for my ears they're more comfortable. I recommend them, and they fared very well on the Amazon.com reviews

Just bought a pair of Shure SE102 ( also known as E2). Cost me $55, really impressed with them. Great sound, very comfy ,much better than the standard apple buds. Wanted a decent pair of sound isolating buds, but have a limited budget, these fit the bill.

Have you tried Acoustibuds? They are really comfortable and they don't fall out all of the time.
I got mine from Amazon for $14.99. See http://www.redferret.net/?p=13897 also,
