ipod constantly disk checks

hi, having problems with my ipod. everytime i connect it to my computer it does a disk check. after the ipod does a check everything's ok. but it doesn't update from itunes. also my entire library has been erased. i bought it in july, thinking of returning it to apple, but have been unable to find any info as to where i should ship it, procedure etc.

Go to www.info.apple.com and choose your product from the drop-down menu. Under 'Top Support Questions' is 'Troubleshoot iPod and Get it Serviced'. Read through the info and if you can't solve the problem, the repair request form is linked at the bottom.
