I find the absolute most reliable way to start an iTunes store account without paypal or credit card info is to either get a hold of a free song code or get yourself a gift card, and during the redeem process you will be allowed to set up an account without payment information.

For some minors, see if your parents will set up a on time allowance for you, this would allow them to make an apple ID for you.

It would appear that it allows easy creation of the ID, but once you agree to the terms in the american store it still wants credit card info. Clicking cancel leaves you not signed in.

To get an account without a credit card really means you have to first invest a bit of cash in a gift card from a brick and morter store that can easily be purchased with cash, and setting up the account with it. It will not let you click done without filling in something in the credit card line.

It's been a while since I set up my paypal account, but depending on the information required to simply get an account, perhaps one could make an empty paypal account and use it to set up the account even if no payment information is yet set up in paypal.... May be age limits in terms, but hey, it's the internet.
